We work with plan-managed and self-managed participants. These are the supports we offer; we shape each roster and service to the participant.

Community Nursing

Registered nurse visits at home for wound care, medication, and clinical monitoring.

Respite & STA/MTA

Short and medium-term accommodation giving participants and carers planned breaks.

SIL

Supported Independent Living — in-home support so participants can live as independently as possible.

Daily Care & Life Skills

Building everyday skills so participants can manage routines and make confident choices.

Personal Daily Living

Hands-on support with washing, dressing, meal preparation, and other everyday tasks.

Life-Stage Transitions

Support through school-leaving, new living arrangements, or other significant life changes.

Transport & Travel

Getting participants to appointments, activities, and community life safely and reliably.

Our intake assessment keeps the paperwork minimal and the process smooth.

In-person visit

We come to you, have a chat, get to know each other and plan together. No rush, no pressure.

Match & plan

We then match you with a support worker and agree on a roster and a support plan.

Step 04

Commence Supports

Then we begin — and we journey together, adjusting as life shifts.

Consistency matters

Same face, same time, whenever possible.

We aim to roster the same support worker for each visit whenever possible. For many participants, that continuity is the difference between tolerating support and genuinely looking forward to it.

And when things change — a hospital stay, a new goal, a family milestone — we adjust and evolve with you.

Do you work with plan-managed and self-managed participants?

Yes — both. We're happy to invoice plan managers directly, or work with self-managed participants and their families.

Can we request the same support worker for each visit?

Absolutely. We'll always try to roster the same person whenever possible.

How fast can supports start?

After an initial in-person assessment, we can typically begin within days.

Can supports be adjusted when life changes?

Absolutely — and we welcome it. Life moves, goals shift, and a plan that suited you six months ago might need a different rhythm now. Just have a chat with us and we'll adjust rosters, goals and support types together with you and your plan manager.
